Assess older adults’ needs and determine are in need of Older Adult Protective Services: Conduct investigations into allegations of abuse, neglect, or exploitation of individuals or property, ensuring thorough documentation and compliance with required policies and procedures. Perform fieldwork, including face-to-face interviews, information gathering, and comprehensive evaluations to determine appropriate services and interventions. Complete reassessments, manage conflict resolution, and provide emergency on-call support as scheduled. Develop and present individualized service plans to support their well-being and independence. Demonstrate professionalism by respecting client autonomy and self-determination while maintaining an unbiased, culturally sensitive, and ethical approach to service delivery. Exemplify the highest level of professionalism

Case loads average 5-8 consumers, but may be as high as 10 consumers. A typical workday will involve reviewing reports and traveling to private residences, nursing homes, personal care homes throughout Cameron, Elk, and McKean counties. Successful employees demonstrate higher order thinking, organization and are dedicated to ensuring the safety of older adults. Applicant must be able to effectively advocate for vulnerable populations and provide required documentation of all findings, contacts and situations involving the case.

Key Tasks and Responsibilities

· Conduct assessments to determine the safety and well-being of individuals in vulnerable situations.

· Thoroughly Investigates reports of abuse, neglect, and exploitation and complete appropriate documentation of discussions and findings

· Assesses the needs of the at- risk individual and develop and implement individual service plans to protect the at-risk individual on an on-going basis

· Collaborate with law enforcement and community organizations to ensure comprehensive support.

· Maintain accurate documentation and case records for all investigations and interventions.

· Serve as a liaison to other social service agencies, hospitals, clinics, courts, and community resources to assist older adults in utilizing these resources to meet their needs

· Consult with professional staff in other program areas to discuss assessments, diagnoses and plans of treatment or support.

· Complete Reports of Needs (RON) for individuals 60 years or older alleging, abuse, abandonment, exploitation or neglect.

· Have strong verbal and written communication skills to interview members of the family, friends, neighbors and others

· Be free of biases and prejudices when interview older adults and collateral contacts

· Be knowledgeable of the Older Adult Protective Services Chapters, policies and procedures, and the Older Adults Protective Services Act and its amendments, along with regulations found in Title 6, Chapter 15 of the Pennsylvania Code, and any related Acts and Department of Aging Regulations for Protective Services for Older Adults.

· Be knowledgeable of local community resources and third-party payors.

· Enter homes unfit for human habitation and cope with clients who want to remain in their homes

· Have strong organizational and time management skills to ensure regulatory standards/timeframes for investigations and documentation are met.

· Attempt the least restrictive interventions during investigations and remain mindful of consumer choice and individual preferences.

· Demonstrate calmness and objectivity in challenging situations.

· Respect Older adults’ autonomy and self-determination

· Develop a good and trusting relationship with the older adult and/or family.

Applicant must:

· Have strong time management skills and cope well under stressful situations

· Be a strong consumer advocate who can maintain a balance of objectivity and empathy

· Have excellent communication skills and quick decision making abilities

· Enjoy working in a team environment providing support to others

· Have advanced computer and documentation skills

· Have a reliable vehicle for work related travel, auto insurance and a valid driver license

· Pass a criminal background check, child abuse clearance and possible FBI clearance

· Pass pre-employment drug and alcohol test

· Must be available to cover on-call hours when required (approx. 6x per year- paid at a premium rate)

This is a full-time 37.50 hours per week position, working 8:00am – 4:00pm with .5 unpaid lunch break. Participation in after hours on-call rotation is required.

Minimum education and experience:

Bachelor's degree in Social Services, Criminal Justice, Nursing, Human Services or a related field AND· 1-2 years experience in mandated reporter incident investigation, assessment, care management, social work, home health, health care, or related environment.

OR

Associate's degree in Social Services, Criminal Justice, Nursing, Human Services or a related field AND· a minimum of 2 years experience in mandated reporter incident investigation, assessment, care management, social work, home health, health care, or related environment.
